1.I agree and understand that by signing this application form I am making an application for an Individual Subscriber Agreement with Cell C Service Provider Company
(Proprietary) Limited ("Cell C SP") and Comm Equipment Company (Proprietary) Limited ("Equipment Supplier") for services and equipment
("Subscriber Agreement"). I confirm that Cell C SP may credit vet this application.
2.I authorise Cell C SP to deduct the payment of the total amount owing by me in terms of this Subscriber Agreement on its own behalf and on behalf of the
Equipment Supplier, by means of payment method selected above. I understand that any change to the payment method and/or the date of the payment
shall be communicated to Cell C SP and I shall be bound by such changes.
3. I confirm that the above package details and subscription information are correct and are my own choice and that the package details will suit my needs.
I confirm that I had the opportunity to consider all available packages. I am aware that I have to pay the applicable subscription fees, the fees for any value
added services, WASP services, third party applications, usage fees and all costs, charges and fees related to the Subscriber Agreement.
4. I confirm that, if requested, I was shown a sample of the equipment of the same make and model of my choice. I had the opportunity to speak to a salesperson
about the functions and features of the equipment and confirm that these meet my specific needs. I confirm that I was given the opportunity to inspect the
equipment, the accessories that come with the equipment and the SIM card. I understand that I will not be entitled to exchange or replace the equipment for a
different make or model and confirm that the out of box failure policy has been explained to me.
5. By signing this application form in the space provided below, I confirm that the above details are correct, that I have read the application form and the terms
and conditions of the Subscriber Agreement in its entirety and accept all confirmations, declarations and authorisations provided by me in this application form.
I further confirm that I had an opportunity to read the application form and the terms and conditions of the Subscriber Agreement, to ask questions and
I understand what the application and the Subscriber Agreement means and the effect it has on my rights.
6. I confirm that the following items have been clearly explained to me and that I am aware and familiar with the terms and conditions relating to these items:
* Using the 24/7 call centre
* Store contact details
* Your bill
* Accessories
* International roaming
* Cell C network coverage
* Migrations
* Setting up MMS, GPRS and WAP
* C-Surance (NOTE: Customer to call and sign up in 7 days if required)
* Debit order date and mandatory 1st debit order
* OBF (within 7 days with proof of purchase), warranty (retain till slip) equipment repair and returns policy
* Once-off connection fee, SIM card fee, management fee, administrative fee
* Setting up MMS, GPRS and WAP
